CareVoyage Essential Plan membership is available for permanent residents of the United States of America (“US”) under the age of 75. An Essential Plan membership includes a Primary Member and up to four (4) additional Family Members. “Family Member(s)” may include the Primary Member’s spouse or domestic partner and unmarried dependent children (including step, foster, and legally adopted children) under the age of 19 (or under the age of 23 if the dependent child is a full-time student in actual attendance at an accredited school or college) who are dependent on the Primary Member for support and maintenance. Subject to the four (4) Family Member maximum, Family Members added during the term of an Essential Plan membership will be covered at no additional charge upon notification to CareVoyage prior to the Primary Member’s initial departure from the Residence Address as defined herein.
To qualify as a dependent child, the Primary Member or the Primary Member’s spouse or domestic partner must be the child’s legal guardian and the child must either be under the age of 19 and reside with the Primary Member or be under the age of 23 and a full-time student in actual attendance at an accredited school or college and be chiefly dependent upon the Primary Member for support.
A Family Member must be traveling with the Primary Member to be eligible for CareVoyage services. CareVoyage will not provide services to a Family Member who is not accompanied by the Primary Member.
Note: The term “Member(s)” refers to the Primary Member and all Family Members.
A Member’s “Residence Address” is the Primary Member’s current home address in the US on file with CareVoyage (identified by the Primary Member during enrollment unless changed by the Primary Member after enrollment). All Family Members must have the same Residence Address as the Primary Member. If a Member’s Residence Address changes during the term of the membership, the Primary Member must notify CareVoyage of the change prior to initial departure on a trip.
A Primary Member wanting to change the identity of any Family Member to be included in an Essential Plan must notify CareVoyage of the change before the Member commences travel from his or her Residence Address.
NOTE: The Essential Plan does not provide medical evacuation benefits. Medical evacuation benefits are available through the CareVoyage Platinum Plan, which is a separate plan available from CareVoyage at additional cost.
As an Essential Plan Member, subject to the Limitations identified herein, while traveling outside the US, CareVoyage will provide qualifying Members with access to:
NOTE: Medical services are subject to availability and location: (1) The Treatment Location must be in CareVoyage‘s service area and (2) The Treatment Location must allow the medical services provider on-site. The Primary Member is responsible for ensuring that the medical services provider is allowed to provide services at a Member’s Treatment Location. CareVoyage is not responsible if a Treatment Location does not allow medical services providers on-site.
By enrolling in CareVoyage, Members acknowledge and agree:
Each adult (over the age of nineteen (19)) who is intended to be included as a Member in any Essential Plan must acknowledge and accept these Terms and Conditions on behalf of himself or herself and any dependent minor Family Members. Failure to do so may result in the immediate termination of membership or prevent a Member from receiving CareVoyage services.
Subject to the Termination provisions identified herein, the “Membership Term” of a CareVoyage Essential Plan membership is one (1) year commencing on the Effective Date.
A membership’s Effective Date is the date and time the Primary Member enrolls and fully pays for a CareVoyage membership.
A “Membership Term” commences on the Effective Date and terminates one (1) year thereafter at 11:59 PM Eastern Daylight Time.
Note: The Membership Term does not commence until CareVoyage collects the full membership fee. A membership is not valid if the membership fee payment is declined, returned, or otherwise unpaid when a Member departs from the US.
Unless canceled by the Primary Member prior to the end of the Membership Term, membership renews automatically for an additional one (1) year term, subject to CareVoyage’s receipt of the renewal membership fee. CareVoyage reserves the right to refuse renewal at its sole discretion.
Note: A Member should confirm membership status prior to commencing travel. If a membership terminates while traveling overseas, CareVoyage services will not be available after the termination date.
To be eligible for CareVoyage services for a specific trip, the Effective Date must be at least seventy-two (72) hours prior to the time a Member departs from the US.
NOTE: During its initial term, a CareVoyage Essential Plan membership is non-refundable and non-transferable. Following an automatic renewal, a Primary Member may cancel the membership within seventy-two (72) hours of renewal and receive a full refund of any renewal fees paid.
CareVoyage reserves the right to terminate or refuse renewal of a membership at CareVoyage’s sole discretion. Should CareVoyage exercise its right to terminate a membership, CareVoyage shall refund the Member a portion of the membership fee prorated based on the remaining term of the membership.
All membership applications and enrollment forms must include accurate information to ensure program eligibility. Any false or inaccurate information that would affect a Member’s eligibility is grounds for immediate termination of the membership.
The CareVoyage Essential Plan provides access to medical services providers. Subject to the Limitations described herein, CareVoyage will connect the Member with a medical services provider that will conduct a Standard Medical Visit at the Member’s Treatment Location within CareVoyage’s coverage areas. CareVoyage is not a healthcare provider. CareVoyage does not provide medical advice, diagnosis, or treatment.
Essential Plan Membership provides Members with access to medical services providers at fixed-rate prices.

For each Standard Medical Visit, CareVoyage will charge the Primary Member’s credit card on file a flat fee. The fee shall be paid in advance when the Member books the service. The Primary Member will be fully responsible for the costs regardless of whether the Primary Member or a Family Member books the service.
Medical treatment beyond the scope of a Standard Medical Visit must be arranged directly between the Member and the Medical services provider. By way of example only, and without limitation, the following are beyond the scope of a Standard Medical Visit:
For treatment beyond the scope of a Standard Medical Visit, the Primary Member will be fully responsible for all costs. CareVoyage will not be responsible for any such costs.
NOTE: Because the Essential Plan is not an insurance policy, all Members will need to maintain their own appropriate health insurance.

1. The Member Pays a Flat Fee for Each Scheduled Standard Medical Visit
For each Standard Medical Visit requested, the Member shall pay CareVoyage a flat fee. Payment will be made by the Primary Member’s credit card on file, and the Standard Medical Visit will be scheduled once payment clears. CareVoyage will pay the provider on the Member’s behalf.
2. The Member Pays all Other Medical Costs Directly to the Medical Services Provider
CareVoyage will not pay or reimburse for testing, imaging, medications, procedures, hospital care, transport, facility-based treatment, or other costs associated with medical care outside of a Standard Medical Visit described herein.
Any care other than a Standard Medical Visit must be arranged directly between the Member and the medical services provider or facility. CareVoyage will not be involved in claims or payments for services other than for Standard Medical Visits.
A Member must contact CareVoyage to cancel or reschedule medical services at least three (3) hours before the scheduled time to avoid being charged for the service.
Providers may occasionally experience reasonable delays. Members who leave the Treatment Location before the provider arrives may not be eligible for refunds.
Subject to the Limitations described herein, to qualify for Essential Plan services, a Member:
Members have a continuing obligation to provide accurate, up-to-date information upon enrollment and throughout the term.
Members must comply with all local laws and safety regulations while traveling.
CareVoyage services are not available to a Member if his or her illness or injury is a result of or is contributed to by the following:

Note: Regardless of whether medical services generally are available in a geographic location, it is the Primary Member’s responsibility to ensure that the medical services provider is allowed access to the Member’s Treatment Location. CareVoyage reserves the right to terminate or refuse renewal of a membership at CareVoyage’s sole discretion. Certain activities by a Member may result in immediate membership termination by CareVoyage. These include, without limitation:

CareVoyage is solely responsible for the interpretation and application of the terms contained in these Terms and Conditions. All determinations by CareVoyage shall be final and conclusive.
CareVoyage does not collect or store protected health information with respect to its Members. A Member may obtain a copy of the CareVoyage Privacy Policy in the following ways:
CareVoyage, at its discretion, may monitor or electronically record communications between its employees or designated representatives and a Member. By participating as a Member, you specifically authorize communications involving you, and to which you are a party, to be recorded and utilized for quality control of other purposes.
CareVoyage and the Member, for himself or herself and any dependent children Family Members, agree that the interpretation of these Terms and Conditions is governed by the laws of the State of New York, without regard to its choice of law rules, and any dispute between a Member and CareVoyage shall be finally resolved by the courts of the State of New York, regardless of whether a third party not subject to jurisdiction in the State of New York is joined in the dispute.
CareVoyage and the Member, for himself or herself and any dependent children Family Members, agree to waive their rights to trial by jury.
Members are encouraged to provide honest, non-incentivized reviews based on their personal experiences.
CareVoyage reserves the right to remove fraudulent, misleading, or inappropriate reviews.
CareVoyage is not responsible for the accuracy or content of user-submitted reviews.
These Terms and Conditions constitute the entire agreement between CareVoyage and the Member with respect to their subject matter and supersede all previous understandings and agreements, whether oral or written. The terms of these Terms and Conditions may not be altered, varied, or modified in any way except as in writing by CareVoyage.

Platinum Plan Members may receive emergency medical evacuation services at no additional cost if hospitalized while away from home and medically eligible for transfer. CareVoyage’s medical evacuation services are available to Platinum Plan Members traveling 150 miles or more from his or her Residence Address.
Subject to the qualifications and limitations described below, evacuation services include:
Transport is subject to availability and clinical appropriateness. All emergency medical evacuations are coordinated and fulfilled by Medjet. All transport decisions are made by Medjet based on clinical assessments and logistical feasibility.
Subject to Medjet’s Rules and Regulations, Members are provided with access to commercial medical escorts and medically dedicated aircraft capable of transporting them from domestic and international hospitals to the hospital of their choice in the US during the term of their membership. In addition, Medjet provides Members with access to medical professionals for consultations and other Member services.
A CareVoyage Platinum Plan membership provides access to authorized affiliates only. All arrangements for medical transport and repatriation will be made by Medjet.
Neither CareVoyage nor Medjet will reimburse Members for expenses they incur on their own. Members will need to maintain appropriate health insurance in their Home Country that will provide for inpatient admission.
EXTENDED STAYS OUTSIDE OF RESIDENCE COUNTRY
A Member traveling, living or otherwise staying outside the US for an uninterrupted period in excess of 90 consecutive days is not eligible for medical evacuation services.
WORLDWIDE REPATRIATION
Subject to limitations on services described herein, when a Platinum Plan Member becomes hospitalized as an inpatient due to illness or injury while traveling 150 miles or more from the Member’s Residence Address, Medjet will arrange for medical transportation and repatriation services to the hospital of the Member’s choice in the US.
Affiliate aircraft used for the medical transport are fully equipped intensive care aircraft staffed with specially trained medical teams. However, if the Member’s condition permits, he or she will be transported by scheduled commercial airline while in the care of a Medjet authorized medical escort.
Medical transport services are available to any Platinum Plan Member who qualifies for medical transport services in accordance with these Terms and Conditions, is hospitalized as an inpatient 150 miles or more from his or her Residence Address, and is accepted as a patient into an available inpatient bed by an admitting physician at the hospital of the Member’s choice in the US.
Medical transport services are not available to a Member with mild lesions, simple injuries such as sprains, simple fractures, or mild illnesses that can be treated by local doctors and do not prevent the Member from continuing his or her trip or returning home without medical attention.
Both the originating and receiving hospitals must be accessible by ground ambulance to transport the Member to and from an airfield capable of accommodating a Medjet authorized aircraft (in the case of a medical transport via medically dedicated air transport) or a commercial aircraft (in the case of medical transport via commercial airline in the care of a Medjet authorized commercial medical escort).
Due to the limited medical facilities and testing available on cruise ships, the Member must be admitted to a hospital on shore before scheduling medical transport to another hospital.
The time frame for medical transport is dependent on affiliate aircraft availability, required permits and visas for the respective countries, and other factors that may be beyond Medjet’s control.
Members must have proper documentation to return to the US. Neither CareVoyage nor Medjet is responsible for obtaining these documents in the event of a request for transport.
Medjet will arrange for medical transport via commercial airline in business class, if available, in the care of a Medjet authorized commercial medical escort if:
One (1) traveling companion may accompany each Platinum Plan Member being transported via scheduled commercial airline, at no additional cost, via economy class.
Medjet will arrange for medical transport via medically dedicated air transport on a Medjet authorized aircraft if:
One (1) traveling companion may accompany each Platinum Plan Member being transported on a Medjet authorized aircraft during a medically dedicated air transport, at no additional cost, provided space is available and the Member’s care will not be compromised.
While Medjet makes every effort to accommodate Platinum Plan Members, due to limited space available on medical aircraft, the Member and any accompanying passenger are limited to one small carry-on bag each.
All arrangements for medical transport and repatriation will be made by Medjet. Decisions regarding the urgency of the case, the best timing and the most suitable means of transportation will be made by Medjet after consultation with the local attending physician.
Medical Assessment – Medjet will require a Medical Assessment in order to determine eligibility for benefits and stability for transport. The Medical Assessment requires a consultation between the Member’s treating physician, who will provide a final or interim diagnosis that the Member will require continued inpatient hospitalization, and a Medjet physician, who will review and evaluate the treating physician’s diagnosis in order to determine the Member’s transport requirements.
A Member must be medically stable for medical transport.
Assuming all other medical transport criteria are met, a Member who is initially considered medically unstable for transport to the hospital of the Member’s choice in the US may first be transported to the nearest appropriate medical facility for initial stabilization. After this initial stabilization, Medjet will arrange continued transport to the hospital of the Member’s choice in the US if the Member continues to meet medical transport criteria.
COVID-19 SPECIALIZED TRANSFER
Subject to all other Terms and Conditions and the following additional limitations on services described herein, when a Member becomes hospitalized as an inpatient due to Covid-19, is more than 150 miles from their Residence Address and requires continued inpatient hospitalization, Medjet will arrange for Covid-19 Specialized Transfer to the Member’s hospital of choice within the US.

Members otherwise eligible for transfer for Covid-19 must not exceed the maximum allowable height, weight and girth requirements set forth by the manufacturers of Covid-19 transport pods utilized in the safe transfer of Covid-19 positive patients. Please contact Medjet if you have questions or concerns regarding the sizing requirements prior to travel.
No traveling companions or family members will be allowed to accompany patients transported for Covid-19.
If a hospitalized Member is under quarantine by a medical facility, governmental entity, or any other regulatory entity exercising jurisdiction over that medical facility, governmental entity, or regulatory entity will not allow transfer, transport will not be possible.
The receiving hospital selected by the hospitalized Medjet Member must agree to accept the patient. Otherwise, Medjet’s transport to that hospital will not be possible.
The time frame for Covid-19 Specialized Transfer WILL BE extended beyond that of typical medical transports and is dependent on multiple factors including, but not limited to, affiliate availability to transfer Covid-19 patients, required permits or permissions, and any other factors that are beyond Medjet's control.
With respect to Covid-19 specialized transfers, to the extent of any actual or claimed inconsistency between the Covid-19 Specialty Transfer provisions and any other provision(s) of the Terms and Conditions, the Covid-19 Specialty Transfer provisions control.
In addition to the Limitations identified in Section VI.B, which are as fully applicable to Medjet as to CareVoyage, the following limitations apply to medical evacuation services provided by Medjet:
Platinum Plan medical transport services are limited to two (2) separate medical transports per membership year, except for repatriation transports involving multiple Family Members requiring simultaneous repatriation. Under these circumstances, each family member will receive one (1) transport.
Due to the high risk of sending registered aircraft and personnel into countries where the US Department of State has issued a travel advisory of level 3 or 4, membership services are subject to exclusion or limitation in these areas. A complete list of travel advisories for every country in the world may be found at the US Department of State’s website. Please contact CareVoyage if you have questions regarding your destination prior to your travel.
Medical transport services are not available to a Member during a period of inpatient or outpatient hospice care.
A Member who is medically discharged from the hospital or leaves against medical advice and is physically able to travel on his or her own is not eligible for Medical transport services for the remainder of the Member’s trip.
A Member with tuberculosis or other chronic airborne pathogens will not be transported, with the exception of Covid-19 as described in the Covid-19 Specialized Transfer Section.
Medical transport services will not be provided to any Member who has a diagnosis of or is suspected of having a Biosafety Class Level 3 (and above) pathogen as classified by either the Centers for Disease Control and Prevention (CDC) or the National Institutes of Health (NIH), with the exception of Covid-19 as described in the Covid-19 Specialized Transfer Section.
A Member beyond 12 weeks’ intrauterine gestation will not be transported, and any Member with any extrauterine pregnancy will not be transported.
Medical transport services will not be provided to any Member with a suspected or diagnosed detached retina, whether before or after surgical treatment.
Medical transport services will not be provided in cases where the Member’s primary admitting diagnosis is an inpatient psychiatric disorder.
Medical transport services will not be provided to any Member hospitalized as a result of the use or abuse of alcohol or drugs (illicit or prescription), including, without limitation, hospitalization for addiction, withdrawal or complications of alcohol or drug abuse.
A Member who is hospitalized at the time of enrollment will not be eligible for transport services for that hospitalization.
A Member on an organ transplant list prior to enrollment will not be eligible for transport for that transplant.
A Member must meet the physical requirements for his or her safe transport on aircraft commonly utilized in the air medical transport industry and Medjet affiliate aircraft.
A Member who would otherwise qualify for transfer by commercial carrier will not be eligible for transfer via medically dedicated aircraft if the Member’s physical size exceeds the maximum allowable weight or girth requirements for transfer via commercial airline. Members otherwise eligible for transfer via medically dedicated aircraft must not exceed the maximum allowable weight and girth requirements for such aircraft and their medical transport providers.
In the event of a Member’s death while traveling 150 miles or more away from the Member’s Residence Address, Medjet will arrange and pay reasonable and customary charges up to $6,000 for the preparation and return of the Member’s remains to the US. These charges will be at the sole discretion of Medjet.
This membership benefit includes:
As soon as Medjet is notified of a Member’s medical situation, Medjet staff will establish communication with both the family and the local attending medical provider, obtain a full understanding of the situation, and begin to monitor the Member’s condition. Medjet staff will stay in communication with local medical personnel and relay necessary information to the Member and, upon request, his or her family or employer until the situation is resolved and either the Member is able to resume travel or a medical transport is initiated.
Members may send and receive emergency messages to and from relatives, friends, and business associates toll-free, 24 hours a day through the Medjet staff.

Members can receive assistance with foreign language interpretation over the telephone when they are having difficulty communicating with local medical specialists by calling Medjet’s toll-free number (800-527-7478) in the US, Canada or the Caribbean, or calling collect from anywhere in the world (205-595-6626).

IF HOSPITALIZED WHILE TRAVELING, HAVE THE FOLLOWING INFORMATION AVAILABLE WHEN YOU CALL CAREVOYAGE OR MEDJET:
For each Standard Medical Visit requested, the Member shall pay CareVoyage a flat fee. Payment will be made by the Primary Member’s credit card on file, and the Standard Medical Visit will be scheduled once payment clears. CareVoyage will pay the provider on the Member’s behalf.
CareVoyage will not pay or reimburse for testing, imaging, medications, procedures, hospital care, transport, facility-based treatment, or other costs associated with medical care outside of a Standard Medical Visit described herein.
Any care other than a Standard Medical Visit must be arranged directly between the Member and the medical services provider or facility. CareVoyage will not be involved in claims or payments for services other than for Standard Medical Visits.
A Member must contact CareVoyage to cancel or reschedule medical services at least three (3) hours before the scheduled time to avoid being charged for the service.
Providers may occasionally experience reasonable delays. Members who leave the Treatment Location before the provider arrives may not be eligible for refunds.
Subject to the Limitations described herein, to qualify for Platinum Plan services, a Member:
Members have a continuing obligation to provide accurate, up-to-date information upon enrollment and throughout the term.
Members must comply with all local laws and safety regulations while traveling.

Note: Regardless of whether medical services generally are available in a geographic location, it is the Primary Member’s responsibility to ensure that the medical services provider is allowed access to the Member’s Treatment Location. CareVoyage is not responsible if a Treatment Location does not allow a medical services provider on-site for treatment.

CareVoyage reserves the right to terminate or refuse renewal of a membership at CareVoyage’s sole discretion. Certain activities by a Member may result in immediate membership termination by CareVoyage. These include, without limitation:

CareVoyage reserves the right to change or amend the terms contained in these Terms and Conditions at any time without prior notice. The most current version is available on CareVoyage’s website.
CareVoyage is solely responsible for the interpretation and application of the terms contained in these Terms and Conditions. All determinations by CareVoyage shall be final and conclusive.
CareVoyage does not collect or store protected health information with respect to its Members. A Member may obtain a copy of the CareVoyage Privacy Policy in the following ways:
CareVoyage and Medjet, at their discretion, may monitor or electronically record communications between their respective employees or designated representatives and a Member. By participating as a Member, you specifically authorize communications involving you and to which you are a party to be recorded and utilized for quality control of other purposes.
CareVoyage and the Member, for himself or herself and any dependent children Family Members, agree that the interpretation of these Terms and Conditions is governed by the laws of the State of New York, without regard to its choice of law rules, and any dispute between a Member and CareVoyage shall be finally resolved by the courts of the State of New York, regardless of whether a third party not subject to jurisdiction in the State of New York is joined in the dispute.
CareVoyage and the Member, for himself or herself and any dependent children Family Members, agree to waive their rights to trial by jury.
Members are encouraged to provide honest, non-incentivized reviews based on their personal experiences.
CareVoyage reserves the right to remove fraudulent, misleading, or inappropriate reviews.
CareVoyage is not responsible for the accuracy or content of user-submitted reviews.
These Terms and Conditions constitute the entire agreement between CareVoyage and the Member with respect to their subject matter and supersede all previous understandings and agreements, whether oral or written. The terms of these Terms and Conditions may not be altered, varied, or modified in any way except as in writing by CareVoyage.
